These notices describe your rights as a patient of Skiatook Family Clinic, how we protect your health information, and our commitment to serving everyone in our community.

Service Notice

  1. We serve all patients, and no one is denied access to services due to their inability to pay.
  2. Discounted/sliding fee schedules for essential services are offered based on family size and income.
  3. We do not deny services to patients based on a person’s race, color, sex, age, national origin, disability, religion, gender identity, or sexual orientation.
  4. We accept health insurance, including commercial insurance, Medicaid, Medicare, and the Children’s Health Insurance Program (CHIP).

Notice of Privacy Practices

Effective date: February 1, 2026

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Skiatook Family Clinic keeps protected health information (“PHI”) private. PHI is any information, in any form, that identifies you and relates to your past, present, or future physical or mental health condition, and it includes your billing records. We may use and share your PHI with other entities to provide health care to patients and for purposes of billing and payment for those services.

We are required by law to maintain the privacy of PHI, to provide you with a notice of our legal duties and privacy practices with respect to PHI, to notify you following a breach of unsecured PHI, and to follow the terms of the notice currently in effect.

Entities covered by this notice

This notice covers all Skiatook Family Clinic employees. It also covers other health care providers who may come to our offices to help care for patients — such as locum tenens physicians, nurse practitioners, and students not employed by Skiatook Family Clinic — unless those providers give you a separate notice.

Uses and disclosures without your permission

  • For treatment: to provide, manage, and coordinate your health care with other providers.
  • For payment: for billing purposes, including sharing PHI with your health insurance company.
  • For health care operations: for quality assessment, evaluating staff, and similar operational purposes.
  • Other permitted disclosures in limited circumstances defined by law, including organ donation, certain research, threats to health or safety, specialized government functions, workers’ compensation, and inmates of correctional institutions.

Uses and disclosures requiring your written permission

We must obtain your written authorization before using or sharing your PHI for any purpose not described in the notice. You may revoke your authorization in writing at any time; the revocation will not apply to disclosures we already made in reliance on it. Marketing communications, sale of PHI, and psychotherapy notes each require authorization, with limited exceptions.

Your rights regarding your PHI

  • Request restrictions on certain uses and disclosures. We are not required to agree, except where you pay out of pocket in full for a service and ask us not to share that information with your health plan.
  • Confidential communications — ask us to contact you a certain way or at a certain location. We will accommodate reasonable requests.
  • Inspect and copy your PHI. A reasonable fee may apply for copies.
  • Request an amendment if you believe your PHI is incorrect or incomplete.
  • An accounting of disclosures we have made of your PHI.
  • A paper copy of this notice at any time.

To ask a question or raise a concern about your privacy, contact the clinic at (918) 396-1262 or 201 East Second Street, Skiatook, OK 74070. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.

Confidentiality of substance use disorder records

Records that identify you as a patient receiving treatment for a substance use disorder receive additional federal protection under 42 C.F.R. Part 2, Confidentiality of Substance Use Disorder Patient Records, in addition to the protections of HIPAA at 45 C.F.R. Parts 160 and 164.

These records cannot be disclosed without your written consent unless the regulations otherwise permit it. You may revoke a consent at any time, except to the extent that action has already been taken in reliance on it. You might be denied services if you refuse to consent to a disclosure for purposes of treatment, payment, or health care operations where state law permits — but you will not be denied services if you refuse to consent to a disclosure for other purposes.

Nondiscrimination

Skiatook Family Clinic does not deny services to patients based on a person’s race, color, sex, age, national origin, disability, religion, gender identity, or sexual orientation. We comply with applicable federal civil rights laws and do not discriminate on these bases in our programs and activities.
