Call the clinic at (918) 396-1262 and we will mail you a packet or have one ready at the front desk. You are always welcome to complete your paperwork here — just arrive about 30 minutes before your appointment.

- We serve all patients, and no one is denied access to services due to their inability to pay.
- Discounted/sliding fee schedules for essential services are offered based on family size and income.
- We do not deny services to patients based on a person’s race, color, sex, age, national origin, disability, religion, gender identity, or sexual orientation.
- We accept health insurance, including commercial insurance, Medicaid, Medicare, and the Children’s Health Insurance Program (CHIP).
